I will like to package a small java application that serve HTML pages using Servlet. Is it possible to bundle this small application to serve dynamic HTML in e-book? In giving more details...java application outputs a question in HTML format with 3-4 answers to select. When user select radio buttons/checkboxes and submit answers it calls java application which serve another question. Can you please tell me if I can to bundle all this in e-book?

You can make an ebook page which uses a FRAME or IFRAME or just a link to the form. The user would need to connect to the Internet to use the pages your server is sending.
Make sure you turn off "Automatically Open Internet links in a new window" in Edit User Interface
