Just view the source and save as your own html file, obviously change the passwordlist = new Array(1001) and your passwords to your own codes, and change other stuff to suit your own needs.
The password list can be generated by your copy of ebook compiler, just save your list as "JavaScript source code" then copy from the generated .js file and copy over the one in the html file. If you have less passwords then 100, you will also need to edit the "<option>" values to the number of passwords you have.
You will also need to change the text box properties to your own message.
