I downloaded your eBook and opened it with no problem. However there doesn't appear to be any images. All of the "buttons" and image "header" images a blank ( place holders).
The previous and next buttons actually work but there is no image for them
Have you placed an image folder in your eBook? If so that is not allowed in Activ E-Book. The only folder that you can keep your material in is the "Source" folder.
